Aging Case Management

Our case management program assists seniors at every point in the spectrum of care, whether they are transitioning back home from a hospitalization or are experiencing an age-related decline in health that will require ongoing support.

Professional bilingual staff (English, Mandarin, Cantonese) provides connections to community care services, such as home-delivered meals, environmental safety checks, and psychological and emotional support. Other  services offered that allow aging adults to “age in place” are:

  • Arranging and monitoring Home Support Services
  • Home-delivered meals
  • Paratransit services
  • Medi-Cal and Medicare counseling
  • Referrals to money management and legal services as well as medical appointments
  • Advocacy and assistance for home-bound individuals

Case management services are available on a sliding scale, based on ability to pay.
